For valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I hereby irrevocably grant to OnStar, its agents, employees, successors and assigns, and anyone authorized by any of them (collectively "OnStar"), the right and license to use my name, voice, likeness, photo, quotations and/or biographical information in any materials or media, without territorial, time, use or any other limitation. I hereby assign and transfer to OnStar all my right, title and interest, including, but not limited to, the right to copyright said materials and the right to renew said copyright anywhere in the world, in any materials utilizing my name, voice, likeness, photo, quotations and/or biographical information.

I hereby release, discharge and agree to hold harmless OnStar from any and all rights, claims, demands, damages and actions which I, my heirs, executors or assigns may have in connection with the use of my name, voice, likeness, photo, quotations and/or biographical information in all materials and media including, but not limited to, any blurring, distortion, alteration, optical illusion, and use in composite or edited form.

I hereby warrant I am of legal age and have every right to contract in my own name without violating any other commitments.

I have read this document in its entirety. I am fully familiar with its contents. I fully understand its contents. I have had any questions answered to my complete satisfaction. I am executing this document voluntarily.
